This form classifies a single AI system with a single intended purpose. One system means one specific tool, product, or application doing one defined job — for example, a CV screening tool, a patient chatbot, or a contract analysis system.
If you have more than one AI system to assess, each one needs its own separate form. This is required by the EU AI Act because risk classification depends on what each system does and how it is deployed — and that is different for each system.
